SRTA PD Fund Evaluation Form 2018-2019

This form is to be submitted after the completion of any event covered by SRTA PD funding (for which your application was pre-approved by the SRTA PD Committee). This form will automatically be emailed to the PD chair.
Please remember to email scanned receipts and one-page articles (if Out-of-Area, University Coursework or Group Project) to pdchair@srteach.org.
